Each Agent must attend and download the CDL Profit Trainer App. The training provides hands-on, behind-the-wheel training so each Agent understands every service type and learns how to better serve their client's unique situation. This fee is non-refundable. You have 90 days from the payment date to activate your Agency account. |

Agency Partnership Activation $3,000 + $7,000 Your $3,000 Enrollment Training Fee is applied toward the Activation Fee, leaving a $7,000 balance. AGENT ACCESS: $1,000 PER AGENT, PER YEAR 10-Agent Minimum = $10,000 annually. This activates a personalized dashboard for each Agent and includes dashboard training. Your Agents can then provide any of the available app services based on each client’s unique situation. Your Agency chooses which services each client receives—and sets its own recurring monthly price. This fee is non-refundable. |

CHANGES TO THESE TERMS
Eye1 may update these Terms and Conditions from time to time. Changes will become effective when notice is provided or the updated terms are posted, as permitted by applicable law.
Your Agency’s continued use of Eye1 services after the effective date of an update means your Agency accepts the updated terms, to the extent permitted by law.
